The above Hess chart shows:-

The right eye is primarily affected, indicated by its smaller field

Underaction of the right lateral rectus (primarily affected muscle)
Overaction of the left medial rectus (muscle sequela)
Overaction of the right medial rectus (muscle sequela)
Underaction of the left lateral rectus (muscle sequela)

Diagnosis: Right Sixth Cranial Nerve Paresis
